Terms of Service — Healthfolio Personal Screening Plan

1. Who we are

Healthfolio is a product of COGNERA LLP, a limited liability partnership registered in England and Wales, No. OC457887. Registered office: 3rd Floor, Suite 207, Regent Street, London, W1B 3HH, United Kingdom. Contact: support@healthfolio.online.

2. What the service is — and what it is not

The Personal Screening Plan is an educational subscription for adults. Based on the answers you give, it produces a personalized list of preventive checks that published clinical guidelines suggest for someone with your profile, a list of tests that are commonly sold but are not indicated for you, a prioritized 14-day action plan, and a summary you can bring to a doctor. Every recommendation carries the source it is drawn from — for example the U.S. Preventive Services Task Force, the CDC, or a professional society guideline.

The service is not medical advice, diagnosis, or treatment, and using it does not create a doctor–patient relationship. It does not tell you whether you have any condition, and it does not predict whether you will develop one. It does not replace a clinical assessment, and it does not decide anything for your doctor — it prepares you to have a better conversation with one. We never provide dosing instructions and never advise you to start, change, or stop any medication.

Do not use the service in an emergency. If you think you have a medical emergency, call 911.

3. Eligibility

You must be at least 18 years old. The service is intended for residents of the United States, and the guidelines it draws on are United States guidelines.

4. Your plan link

Your plan is accessed through a private link tied to your email address. Keep the link confidential: anyone who has it can view your plan. We may add account protection features over time.

5. What you can buy, and how billing works

Available plans and pricing are shown at checkout before you pay.

5.1 Personal Screening Plan (subscription). $1 for your first 7 days. Unless you cancel before those 7 days end, your membership then continues at $29 every 4 weeks. The plan renews automatically and your saved payment method is charged $29 every 4 weeks until you cancel. The exact amount and date of your next charge are shown before you subscribe and again on your billing page at any time, and we email you a reminder before each renewal charge.

5.2 One-time report. A single report is also available as a one-time payment of $29. It is not a subscription, it does not renew, and there is nothing to cancel.

5.3 Price changes. If we change prices, we will notify you at least 30 days in advance; changes apply from your next billing period.

5.4 On your statement. Charges appear as HEALTHFOLIO.

6. Cancellation

You can cancel anytime, online: open your plan page → Manage subscription → Cancel. It takes two clicks and does not require emailing us. Cancellation takes effect at the end of your current paid period, and you keep access until then. We do not provide partial refunds for unused time, except as described in the Refund Policy or where required by law.

7. Refunds

See our Refund Policy. In short: if you are not happy, email support@healthfolio.online within 14 days of your first payment and we refund every charge in full.

8. Your health information

To build your plan, we process the answers you give in the quiz and in the product: your age and sex, height and weight, symptoms you select, conditions a doctor has mentioned to you, procedures you have had, cancer and cardiovascular history in your family, when you last had various checks, and — if you choose to upload them — results from lab reports you provide. You are never required to upload anything.

We do not sell this information. We do not use it for third-party advertising. We never share your health answers, your uploaded results, or anything derived from them with advertising platforms such as Meta or Google, in any form, under any label. 9. Acceptable use

Don't misuse the service: no unlawful use, no scraping or reselling content, no attempts to break or probe the service, no sharing your private link publicly.

10. Intellectual property

The service and its content belong to COGNERA LLP or its licensors. Your plan is for your personal, non-commercial use.

11. Disclaimers and limitation of liability

The service is provided "as is" and "as available". Guidelines change, sources can contain errors, and educational content is not a substitute for professional care. Decisions about your health should be made with a qualified clinician. To the fullest extent permitted by law, COGNERA LLP's total liability for any claims relating to the service is limited to the amounts you paid us in the 12 months before the claim. Nothing in these terms limits liability that cannot be limited by law, or your non-waivable consumer rights.

12. Governing law

These terms are governed by the laws of England and Wales. If you are a consumer, you keep any mandatory protections of the laws of your state or country of residence. Before any formal dispute, contact us — most issues are resolved quickly by email.

13. Changes

We may update these terms; material changes will be announced in the product or by email. Continued use after changes take effect means you accept them.
